  • Welcome to the Final Year Community Programme

    Thank you for expressing interest in the Final Year Community Programme! It is great that you are considering undertaking the highly successful final year programme with the Team here at Birmingham Community NHS Foundation Trust (BCHC). We have some amazing career opportunities, if you aren't familiar with the Trust please take some time to explore our Trust website 


    We are seeking Adult and Learning Disability Field students entering their final year of study in  September,  to spend this 'whole' year with us, in services aligned specifically to your field of nursing.

    September Intake- Students from Adult and Learning Disabilities fields- can apply (BCU,UCB, BNU, UoB and Aston) 

    To be part of this you should have a keen interest in gaining exposure to all that our fantastic Teams have to offer.

    Specific programmes are run in each of our highly specialised divisions. Please read the Field specific Information in the topics below.


    All students on the programme will be able to attend Pathways within the Trust, to for example, our Corporate Division/ Services as well as specialist services to enable you to gain a real understanding of the workings of a large Community Trust and the wider National Health Service (NHS).


    During the programme you will be supported by our Professional Development team who will work with you to ensure that your final year of practice is an exciting and rewarding one.


    All students who access the programme will have a potential job offer if a suitable vacancy arises during the programme in the Division that has been selected (subject to successfully passing your programme).

    Numbers are limited so please make contact as soon as possible.  Allocation to the programme- locality/team- will be decided fairly and take into consideration all requests submitted.

    • Final Year Programme- Adult Field

      Adult Field Students can choose between the following:

      Adult Specialist Rehabilitation (ASR) Services

      The services include:

      • Assessment and Treatment
      • Inpatient Services
      • Nutrition
      • Pain management
      • Prison Health Services
      • Single Point of Access
      • Specialist rehabilitation services

      The Adult Inpatient Wards and Units are located across the city of Birmingham at Moseley Hall Hospital, West Heath Hospital, CU27– Good Hope Hospital, Perry Tree Centre, Annmarie Howes Centre  and HMP Winson Green.

      Or

      Adult Community Services (ACS);

      The services include:

      • Chronic Kidney Disease (CKD)
      • Continence
      • Cardiac Services
      • Dementia Support
      • Diabetes Service
      • District Nursing (Part of IMTs)
      • Integrated Multidisciplinary Teams (IMT)
      • Lymphoedema Service
      • Parkinsons
      • Early Intervention Care Team (EICT)
      • Respiratory service
      • Sickle Cell and Thalassaemia
      • Tissue Viability Service
      • Therapy Hubs

      The District Nursing Teams are based across the City and pathways are available with the various specialist services within the Division.

      Please note- For a Substantive post offer, this division has assessed the role requires you to be a car driver



      • Final Year Programme- Learning Disability Field

        Learning Disability (LD) Field Students will have an opportunity to experience:

        The services include:

        • Community Nursing Services
        • Older Adults
        • Community Intensive Support Team 
        • Community Forensic Team 
        • Single Point of Access
        • Nutritional Nursing Services
        • Health Facilitation Team
        • Respite unit
        • Epilepsy Team
        • Intensive Support Team
        • Day services and Short Breaks
        • Discharge Hub
        • End of Life and Physical Health
